A deadly UPS cargo plane crash occurred shortly after takeoff from the Louisville Muhammad Ali International Airport in Kentucky on Tuesday, November 4, 2025. The catastrophic incident, which involved a McDonnell Douglas MD-11 freighter, has resulted in a significant loss of life and a massive response from emergency services. The plane was bound for Daniel K. Inouye International Airport in Honolulu, Hawaii.

The aircraft, UPS Flight 2976, crashed around 5:15 p.m. local time, erupting into a huge fireball and sending thick plumes of black smoke into the sky. Reports indicate the death toll has risen to at least seven people, with Kentucky Governor Andy Beshear stating that he expects the number to grow as investigations continue. Among the deceased, four people were confirmed not to be onboard the aircraft. Additionally, at least eleven people were injured, with some sustaining "very significant" injuries. The three crew members aboard the plane have not all been accounted for. The crash site is in an industrial corridor near the end of the runway, impacting at least two businesses, including a recycling center and an auto parts store.

Video footage circulating online and from local news sources appeared to show flames on the plane's left wing and a trail of smoke as it attempted to lift off. Aviation experts have suggested this points toward a possible severe fire or an uncontained engine failure during the takeoff roll. The sheer violence of the crash and the resulting fire, fueled by the large amount of fuel onboard for the long flight to Hawaii, complicated the immediate rescue and recovery efforts. Officials have confirmed that the plane was not carrying any hazardous cargo that would create environmental issues.

The National Transportation Safety Board (NTSB) and the Federal Aviation Administration (FAA) have launched an investigation to determine the official cause of the crash. The Louisville airport, home to UPS's global air hub known as Worldport, was shut down following the incident, resulting in the cancellation of flights and a temporary shelter in place order for the surrounding area. Authorities have urged the public to stay away from the active scene due to the remaining danger from fire and debris, and to report any scattered debris without touching or moving it. A family assistance center has been established for those seeking information about loved ones. The focus of the investigation will center on the initial fire and the aircraft’s struggle to gain altitude after the left wing ignited.
